It's actually quite simple ..... Let's take your own example with the Milan match and make ourselves (AH -0.25) on Milan.

First choose your cover style and then in "odds to win" you write the odds offered on Milan win at your bookie and in "cover bet" you write odds for the draw ..... You then write how much your total stake is (lets say 100€) and the calculator will then estimate how much of your total stake in % you should put on Milan and how much you should place on draw ..... With the odds you suggested above you should put 84.85% on Milan and 15.15% on draw and you will then get (AH -0.25) at odds 2.04

Of course doing this you will need to have a good amount of bookies in your portfolio (I have 18-20 trustworthy ones myself) as you will need to get the best possible price to get the best odds on your handicap.
